It is typically more affected by energy restriction than by protein deficiency.
Since RBP is relatively unaffected by the acute inflammatory response, RBP appears to be a more reliable indicator of nutritional status following surgery or injury.
Serum RBP is altered in relation to metabolic syndrome (MetS), a constellation of metabolic abnormalities including central obesity, dyslipidaemia, elevated blood pressure, and hyperglycaemia (1). MetS is associated with an increased risk of type 2 diabetes mellitus and cardiovascular disease (CVD) (2, 3).
RBP is freely filtered by the glomerulus and subsequently reabsorbed and catabolized by the proximal tubule.
RBP is a highly sensitive indicator of renal tubule dysfunction and injury of various aetiologies, including antibiotic and environmental toxicant exposure.
It should be noted that serum RBP levels are depressed in vitamin A deficiency, and urinary levels may theoretically yield a false-negative result in this setting.

Reference

Grundy SM, Cleeman JI, Daniels SR, Donato KA, Eckel RH, Franklin BA, Gordon DJ, Krauss RM, Savage PJ, Smith Jr SC, Spertus JA, Costa F 2005.
